The power of a quick edit - Glenn Cassie

Professionalism is expressed through treatment, but also through writing. In fact, written communication defines the modern businessperson whether through social media or blog posts, website descriptions or promotional articles. The biographies doctors write about themselves, as well as their social media posts and promotional articles, impart to patients/prospective patients the characteristics of a practitioner’s focus.

In this short informational video, basic Canadian style and commonly misunderstood spelling and grammar rules are reviewed. The tips and suggestions will provide clarity on and ideas to improve promotional text. The Power of a Quick Edit is eligible for 30 minutes Category D CE.

Neutralizing Clinical Language for 2SLGBTQIA+ Inclusivity with Dr. Cyndi Gilbert, ND

Neutralizing Clinical Language for 2SLGBTQIA+ Inclusivity with Dr. Cyndi Gilbert, ND.

As licensed health professionals, naturopathic doctors have a responsibility to provide care that is free from discrimination on the basis of assigned sex, gender identity, gender expression, and sexual orientation. This presentation will explore common heteronormative and cisnormative language in clinical practice and provide practical guidance on neutralizing language in clinical and research settings to minimize experiences of enacted stigma in healthcare environments. We will explore evidence-informed best practices on how to take an inclusive reproductive and sexual health history, how to talk about puberty, and how to discuss physiology, anatomy, and behavior without resorting to gendered and/or heteronormative assumptions and shortcuts. Examples and clinical scenarios will be reviewed in the context of jurisprudence guidance and human rights legislation regarding discrimination and harassment. By the end of this session, participants will feel more confident in their abilities to offer an inclusive and 2SLGBTQIA+-competent practice.

Instructor Brief Bio: Dr. Cyndi Gilbert, ND is a naturopathic doctor, author, and faculty member at the Canadian College of Naturopathic Medicine. As a health care provider with a focus on mental wellness, trauma, and 2SLGBTQIA+ health,
she regularly bears witness to the health impacts of social determinants and experiences of discrimination. She advocates for a collaborative, anti-oppressive, and harm reduction approach that centers patients' voices and experiences. Cyndi facilitates anti-oppression training for naturopathic doctors and supervises at the free naturopathic teaching clinic at Parkdale Queen West Community Health Centre.

ADDITIONAL NOTES:

Slide 7 - i believe i was discussing the Ciprodex formulation itself, containing ciprofloxacin & dexamethasone.  Below is the slide note...

Cefuroxime & Cefprozil – G2 cephalosporin (recall: G1 more coverage for Gm+ vs. G4 more coverage for Gm- organisms)

Amoxicillin covers a wide variety of gram-positive bacteria, with some added gram-negative coverage compared to penicillin. Similar to penicillin, it covers most Streptococcus species and has improved coverage of Listeria monocytogenes and Enterococcus. * Amoxicillin has narrow spectrum & is inexpensive

Amoxi-clav for resistant organisms; NB – increased pneumococcal & H. influenzae resistance makes Septra a less desirable agent

A topical fluoroquinolone/dexamethasone preparation (eg Ciprodex) is suggested for uncomplicated AOM with chronic TM perforation or ventilation tubes- presentation is that of painless d/c

Ciprodex has 3mg ciprofloxacin & 1mg dexamethasone per mL

Beta-lactam antibiotics include penicillins, cephalosporins and related compounds. As a group, these drugs are active against many gram-positive, gram-negative and anaerobic organisms. Evidence in the literature supports the selection of amoxicillin as first-line antibiotic therapy for acute otitis media. Alternative drugs, such as amoxicillin-clavulanate, trimethoprim-sulfamethoxazole and cefuroxime axetil, can be used to treat resistant infections.

Penicillin V remains the drug of choice for the treatment of pharyngitis caused by group A streptococci. Inexpensive narrow-spectrum drugs such as amoxicillin or trimethoprim-sulfamethoxazole are first-line therapy for sinusitis. Animal and human bites can be treated most effectively with amoxicillin-clavulanate. For most outpatient procedures, amoxicillin is the preferred agent for bacterial endocarditis prophylaxis. Beta-lactam antibiotics are usually not the first choice for empiric outpatient treatment of community-acquired pneumonia.

Low dose computed tomography (LDCT)For adults aged 55-74 years with at least a 30 pack-year* smoking history who currently smoke or quit less than 15 years ago, we recommend annual screening with LDCT up to three consecutive times. Screening should ONLY be carried out in health care settings with expertise in early diagnosis and treatment of lung cancer.
Weak recommendation; low quality evidence.*pack-year defined as the (average number of cigarette packs smoked daily) x (number of years smoking)

For all other adults, regardless of age, smoking history or other risk factors, we recommend not screening for lung cancer with LDCT.
Strong recommendation; very low quality evidence

Chest x-ray (CXR)We recommend that chest x-ray not be used to screen for lung cancer, with or without sputum cytology.
Strong recommendation; low quality evidence.

The Pan Can Lung Cancer Risk Prediction Model is used to determine if a person should undergo annual computed tomography (CT) screening to detect early-stage lung cancer.

Recurrent vs Reinfection vs Relapse UTI

Recurrent: Two uncomplicated UTIs w/in 6 months OR three or more + urine cultures in the prior 12 months

Reinfection: occurs after 2 weeks of completing antimicrobial therapy; caused by different organism, most common

Relapse: Occurs w/in 2 weeks of completing antimicrobial treatment; caused by the original organism; bacteruria persists during tx or recurs after 1-2 weeks of finishing Abx tx; in minority of women (5-10%)

Dr. Paul Anderson, ND. In this session Dr. Anderson uses his many decades of injection experience to discuss customizing parenteral nutrient combinations and specific dosages for maximum efficacy. While this presentation covers the pharmacology and administration of vitamin, mineral and amino acids it also includes appropriate drug agents.
IV and IM administration are discussed and areas of augmentation effecting
adrenal, mitochondrial, neurological, immune and many other functional areas are also
discussed. Clinical comparisons of oral and injection therapies (and how to augment
therapies orally between injections) are provided. This session has ample
time for Q&A as well.

Pharmacy Concerns In Geriatrics

Dr. Alethea Fleming, ND. A commonly taught, but infrequently observed tenant of geriatric medicine is when a new problem arises first look to the patient’s medication list. Complications arise
with medications more readily in older patients. This lecture widely covers issues in
pharmacy with geriatric medicine using many clear case examples. We briefly
review essentials like how pharmacokinetics and pharmacodynamics change with
age and best tools to help decide how to change or stop medications (including
STOPP and START, Beer’s criteria, pharmacogenetic testing and clinical guidelines).
One primary component is on safe approaches to deprescribing including how to
initiate weaning medications, the importance of harm reduction, the role of OTC
medications, as well as how to best approach communication with patients, families,
and other health care providers. Special issues including changes in sleep with age
and common medication issues as well as the role of pharmacy in dementia are also
addressed. The goal of this presentation is to enable practitioners to feel more
confident in multiple aspects of pharmacy in geriatric care, especially deprescribing.
